Where to find your Invoice Number

On your statement: Bottom-right in the “Invoice Details” box.

By phone: Call us — we’ll verify your identity and provide it.
Where to find your Account Number

On your invoice: Look near “Patient Account #” (often mid-page under the payment box).

By phone: We can confirm after identity verification.
Where to find your Practice Name
- Statement header (top of the page).
- Appointment reminders or visit summaries.
- Clinic signage/website from your last visit.
If you saw more than one provider, use the clinic/department name printed on your statement.
Use your Legal Name
Enter your name exactly as it appears on your statement (first, middle initial if listed, last). If a parent/guardian is the account holder, use the account holder’s name.
